Shared Prize Forms Available

For anyone wanting to distribute their prize winnings with a co-owner, friend, wife or anyone else we have the form available. Please email me at
[email protected] to receive the form. All forms must be submitted to me or faxed directly to STATS (the number is on the form) by Friday, Oct. 12.

If prize is $650, does a W9 need to be filled out? Otherwise I'll just let my teammate accept full amount and pay me.

Yes. Anything $600 or more needs a W-9.
