New mailing address for Michael Zukowski, p.o.box 943, centerville, ma 02632. also I haven't received my signed meat card, could you send that along with a w-9.

Thanks Mike. We'll make the change. As far as the Meat Loaf card we'll let Dave Cushard know you still haven't received it yet since he's handling all of that.

thank you Hey Mike - got the note and I have the new address now. I haven't sent out any of the Meat Loaf card prizes yet but will do so in a batch when the season ends in a few weeks.
I'll announce here and via email that they are on the way shortly after the season is over!

Still looking for a W-9 to sign. I can pull one from the IRS web site-- where would it be mailed or faxed to?
