If you want a shared prize form to distribute your NFBC winnings please contact me at [email protected] and I'll send you the form. The form needs to be filled out, notarized and emailed back to me by Monday Oct. 12. Thanks.

Does this need to be done every year for long time partners where you have these forms on file? Just curious.

Yes, new forms are needed each year since the prizes being won are often different .
