If you want a shared prize form to distribute your NFBC winnings please contact me at [email protected] and I'll send you the form. The form needs to be filled out, notarized and emailed back to me by Monday Oct. 9. Thanks.

This is for if we only want a portion of our winnings to be distributed to us, correct?

Mike, the primary owner receives prizes in full unless this form is submitted. If you want to share the prizes with anyone you need to submit this form. Hope this helps.

My error; I thought this was for partial distributions and keeping the rest of the winnings "on account" for next season (as you or Greg previously discussed).
